"Question: My 12-year-old male cat was just diagnosed with a low-level spindle cell malignant tumour on his ankle/heel area (the tarsus, I believe it’s called). The lump appeared quite quickly, about four weeks ago. He seems healthy otherwise.
The next step is to meet with an oncologist. What is the prognosis with this type of tumour? If we opt for surgery, could the cancer reoccur soon after? We understand that surgery could require amputation, which clearly would not be a desirable outcome; therefore we might decide not to choose surgery. Are radiation and chemotherapy effective and worthwhile for this type of tumour?
We’ll have to make many decisions in the upcoming weeks and are educating ourselves now before meeting with an oncologist. This is our first experience with cancer in a pet.
Answer: Spindle cell tumours belong to a category of tumours known as soft tissue sarcomas. This group includes several kinds of tumours that are named after the type of cell involved and they include fibrosarcomas (connective tissue), hemangiosarcomas (blood), liposarcomas (fat) and several others, including spindle cell tumours. Some sarcomas, called vaccine-associated sarcomas, have been associated with vaccinations.
As you witnessed for yourself, spindle cell tumours can grow very rapidly and very often the only symptom is a firm mass noticed by the owner. You are fortunate that the tumour was diagnosed as early as it was because early detection is important to a successful outcome.
Treatment and prognosis (i.e. will the tumour reoccur after treatment) depend on how advanced the tumour has become and how aggressive it is. This is done by grading the tumour (either Grade 1 or 2) and then customizing the treatment accordingly. X-rays, ultrasound or MRI scan can be helpful with the grading process but the oncologist will rely heavily on the report from the histopathologist who grades the tumour based on a microscopic examination of a biopsy sample.
Once graded, the course of treatment will become clear. Surgery is usually the treatment of choice. If complete excision of the mass is not possible, you will be presented with two options: amputation of the affected limb or surgical excision along with chemotherapy, radiation therapy or both. Chemotherapy and radiation therapy are usually recommended when complete excision of the tumour is not assured, or they may be included in order to slow down tumour growth or prevent the tumour from returning.
You should know that as a rule, cats respond well to chemotherapy. However, if the tumour has metastasized (i.e. spread to other parts of the body), the prognosis is poor. If amputation is required, you should consider it as a reasonable alternative. Cats (and dogs) function quite well with only three legs and can lead happy active lives despite the loss of a limb."
My cat had a spindle cell tumor in his armpit and the vet removed his leg. He did not seem to notice that his leg was gone and lived 11 more years.
